NeuroHealth Case Management Services
NeuroHealth provides bespoke case management services to adults and children across the UK. We specialise in assisting those who have sustained a brain injury, spinal cord injury, amputation or are affected by other trauma.
Restoring Health, Quality of Life & Independence
For our injured clients, our aim is to restore the individual’s function, health, quality of life and independence as close as possible to their pre-injury levels. Of course, that is not always possible, and in those instances we ensure that the injured client is adequately supported and has access to all the services they require to live as fulfilled a life as possible.
Taking an Individual Approach
We also provide services to those with chronic or terminal illness, usually as a result of clinical negligence or industrial injury. As with all our clients, our focus is on quality of life and ensuring that the individual’s needs are met as well as those of the family, who will often be involved in providing a caring role.
What is Case Management?
Case management is a process which promotes recovery, function, independence and wellbeing, through accessing the most appropriate services, whether that is therapy, care, medical or surgical intervention, support or practical assistance. We recognise that every client’s situation and difficulties are unique and we often need to adopt a creative approach to solving problems.
At the outset of our involvement we complete a thorough face-to-face assessment with our client and if appropriate, their family, to determine their current level of need. The findings are detailed in an extensive report which details all of the recommended interventions. We take a holistic and proactive approach and this is reflected in our recommendations.
